Question: Have you ever made a fatal mistake?

No, fortunately I have never made a fatal mistake. But, I cannot speak for the nuclear industry as a whole or any other industry for that matter. Accidents always happen and humans are not perfect.
In my company, we try to promote a nuclear safety culture and we place safety as a top priority in everything we do. It doesn’t mean bad things will never happen, but we will try to keep it to an absolute minimum.

The question asked about fatal mistakes – to me this is where someone got hurt or worse.
