This is a really good question. It was a design safety concern that was identified. On a waste plant, we had designed a route which people stopped taking because it was easier to take a short cut and arrive faster. It wasn’t an issue till it was reviewed that when we needed to escape from the area the route would be insufficient as an escape route. I had to work with the design safety engineers to solve this issue. Thankfully it was resolved.
We had a problem on a new site recently where the electricians didn’t follow the design and had installed lots of large cables in one duct. This caused them to get dangerously hot because there was not enough air around to cool them. We had to get the cables pulled back out and reinstalled in lots of different ducts.
