      That something I design hurts whoever is operating it.

      doing something which could ruin other engineer’s hard work and set the company back weeks in delays. Thankfully it hasn’t happened yet.

      That some day we will come across a problem that we can’t solve before it destroys the world. There are several films about this sort of scenario, asteroids hitting the earth, extreme weather, alien invasion etc. Not sure about the aliens, but the other two are completely possible.
