  None! Thankfully!
  
  I’ve seen things go badly wrong! but generally we work in a very safe manner! Safety is paramount in the energy business!

  I haven’t had any at work. I did once get hit by a motorbike when I was cycling TO work, but that had nothing to do with work, more to do with cycling in London being crazy dangerous! I had a helmet on and there was no major damage and I am still here to tell the tale :).

  None – thankfully! Although health and safety is paramount to good engineering – so perhaps is not surprising either

  Simulations do sometimes blow-up mathematically, but there’s no loss of life.
