I have experienced people removing safety interlocks (mechanical or electrical which protect people from voltage or machinery). We rectified the issue immediately, then did a process called Route Cause Analysis to find out why the safety interlocks were removed. This showed that people did not understand the risk of removing the interlocks. We re-trained people, explaining the risks.
Waste in the lab from other peoples samples. It’s extremely difficult to deal with chemicals when you don’t know exactly what it is! I have dealt with it all though by asking other peoples opinions. I got rid of of loads of old samples so our lab doesn’t have so much rubbish anymore.
I didn’t have to face anything really dangerous. I did some acoustic tests once and the noise was so loud that we needed ear defenders or we could get permanent damage. Then everytime I go to the lab there are basic Healthy&Safety procedures I have to comply with, for example, wearing goggles, gloves to handle parts and other hazardous materials, wearing solid shoes.
