Question: What was the worst facility you've visited ?

I went to a horrible customer site in Germany once where they drove around the place with molten iron in huge buckets on fork lift trucks. They drove so fast that it would spill all over the place when they went past (molten iron is veerrrry hot!!) so it was not very safe at all.
Though it was next to a Ferrero Rocher factory so it did smell nice!
